Revised Membership

Due to the recent changes in law, Conscious Care Cooperative has revised their membership application to make sure everyone is in compliance with WA state laws. On your next visit, we will be asking you to fill out this membership application and will also need to make a copy of your WA state photo ID and original doctor’s recommendation.
